New Patient Information
The Stein Center for Complete Dentistry looks forward to meeting all your oral health needs. To prepare for your first appointment, please fill out the ‘New Patient Forms’ and bring them with you. We will also need a valid ID and your dental insurance information. Your dentist will perform a full mouth evaluation and may order x-rays. One of dental hygienists will clean your teeth. Then you will meet one on one with Dr. Stein or Dr. Weiss for a dental consultation. They may recommend further treatment to benefit your oral health.
